Proxy baptism performed at the Salt Lake Temple on 5 May 1976. This was found on IGI v5.0, FHL film, Batch no. 7604301, Sheet 67, Call no. 1058264, From patron sheets submitted 1969-1991 for LDS temple ordinances.
